• No refunds given once impressions and records are taken for 3D animation stage
• Duration and number of treatments may vary by person
• This service may not be reimbursed by your health fund
• Any additional treatment identified as necessary before undergoing treatment will incur an additional cost
• Patients must be free of tooth decay and gum disease prior to the treatment
• Where patients have not had an examination by a dentist within the last 6 months, a tooth decay and periodontal exam may incur an additional fee at the discretion of Bentleigh Dental Care
• Valid for standard Invisalign® treatment with up to 26 aligners; additional aligners will incur a fee • Mid-Course correction and refinement, if needed, incurs an additional cost
• Lost or broken aligners will incur a fee
• Upper and lower retainers must be worn after Invisalign® treatment or teeth will relapse. 2 retainers will incur an additional cost of $450
• Patients may be referred to a radiologist for additional X-rays to determine eligibility for procedure; covered by Medicare or private health insurance - otherwise surcharge applies
